Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 02/08/21 and concluded on 02/18/21 The director was contacted by telephone to initiate the inspection. There were 15 children present and 4 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a record review the center did not ensure all required information was maintained for each enrolled child.
Evidence:
1. Child #1, enrolled 03/09/20, did not have completed addresses for the required 2 emergency contacts.
2. Child #2, enrolled 09/28/20, did not have the name, phone number or place of employment and phone number for the child's father.

Plan of Correction: The following corrective action was provided by the director during the exit interview:
Child #1's emergency contacts will be updated. The director will follow-up with child's #2's mother to obtain the missing information.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition.
Evidence:
1. In the girl's bathroom. There was peeling paint on the walls of one of the bathroom stall walls.
2. In the boy's bathroom the vent in one of the stalls was rusted.
3. In the two year-old room there was a 2 shelf cubby that was not secured, posing a tipping hazard to the children in care.

Plan of Correction: The following corrective actions was provided by the director during the exit interview:
The center has plans to remodel the bathrooms to include painting the walls and replacing the rusted vent.
The cubby in the 2 year-old room will be moved and secured.
