Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 2/16/21 and concluded on 2/19/21. The director was contacted by email to initiate the inspection. There were three children present and two staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children?s records and one staff record submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of two children's records, it was determined that the facility did not ensure that all required elements were present in each child's record.

Evidence:
1. The record for child #1, who was in care at the time of the inspection, was missing the father's work telephone number and the address for one of the two emergency contact persons.
2. The record for child #2, who was in care at the time of the inspection, was missing addresses for the two emergency contact persons.

Plan of Correction: Center director will contact the parents to obtain missing information. Center director will also review all children's records to ensure that they are complete.

Standard #: 22VAC40-191-60-B
Description: Based on a review of one staff record, it was determined that the facility did not ensure that staff completed a sworn statement or affirmation prior to employment.

Evidence:
Staff #1 (date of hire 1/5/21), who was on duty with the children at the time of the inspection, had a sworn statement or affirmation on file that was dated 2/10/21.

Plan of Correction: Center director will assure that staff members sign a sworn statement or affirmation prior to employment.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of one staff record, it was determined that the facility did not ensure that staff had a central registry finding (CPD check) on file within 30 days of employment.

Evidence:
Staff #1 (date of hire 1/5/21), who was on duty with the children at the time of the inspection, did not a completed CPS check on file.

Plan of Correction: Center director will ensure that CPS checks are submitted within seven days of employment and that they are on file within 30 days.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of one staff record, it was determined that the facility did not ensure that staff had completed fingerprints on file prior to employment as required.

Evidence:
Staff #1 (date of hire 1/5/21), who was on duty with the children at the time of the inspection, did not have fingerprints on tile at the time of the inspection.

Plan of Correction: Center director will ensure that all staff obtain fingerprints prior to beginning employment.
