Emergency Contacts shall be different individuals from the child's parents.

Comments:
This inspection was conducted by licensing staff using an alternate protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 02/10/2021 and concluded on 02/16/2021. The director was contacted by telephone to initiate the inspection. There were no children present as the program has been closed since 09/25/2020. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed 1 staff record and 2 children's records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

The licensing inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

The plan of correction for the violations cited were not received as of 02/23/2021 and will not appear on the violation notice.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to ensure that children's records contain all of the required elements.

Evidence:
1. The enrollment record for child 1 lacked the following required information:
a. Work phone number and place of employment of each parent;
b. Name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached;
c. First date of attendance.
2. The enrollment record for child 2 lacked the following required information:
a. Work phone number for one parent;
b. Work phone number and place of employment for the second parent;
c. Name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached;
d. First date of attendance.

Standard #: 22VAC40-185-90--A
Description: Based on record review and interview, the center failed to ensure that the following statement is included in the written parent agreement that is required to be in each child's record before the first day of attendance:
*A statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.

Evidence:
1. The written communicable disease statement in the record for child 1 and child 2 is incomplete and does not include all required information.
a. The statement on the enrollment form reads as follows: "I agree to inform the YMCA child care staff/director within 24 hours or the next business day by the State Board of Health, except for life threatening diseases which must be reported immediately."
2. The statement lacks the following required information:
".....after his child or any member of the immediate household has developed any reportable communicable disease...."

Description: Based on observation and interview, the center failed to ensure that certain documents related to the terms of the license are required to be posted on the premises of each facility. These are the findings of the most recent inspection of the facility.
