Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was completed on February 22, 2021. The provider was contacted by telephone (video call) to initiate the inspection. There were 35 children present and in the care of eight staff members. The inspector was able to view all required documentation. A virtual tour of the children in care was conducted. Three staff records and three children's records were reviewed. Information gathered during the inspection determined a non-compliance with applicable standards or law and the violation was documented on the violation notice issued to the facility.

The Licensing Inspector reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on review of three children's records and verification from the director, the center failed to obtain documentation of additional immunizations once every six months for one child under the age of two years.

Evidence:
1. The record of Child #3 did not contain all of the required updated immunization records showing what vaccines he had received and that he was adequately immunized by his second birthday. The child's date of birth is 11/10/2016 and his date of enrollment was 02/01/2017. The initial immunization record was received on 01/09/2017 and follow-up records were received on 03/07/2017 and 11/13/2017.
2. The director confirmed not all of the updated immunization records were in the child's file.

Plan of Correction: The director will reach out to the parent of the child and ensure his updated immunization records are obtained.
