his inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on February 19, 2021. The director was contacted by telephone to initiate the inspection. There were 101 children present and17 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 5 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-185-160-C Description: Based on record review, in one of five staff records reviewed, the licensee did not ensure that staff resubmit tuberculosis (TB) test results every two years.
Evidence: Staff #1 TB test results was more than two years old . Staff #1TB test results dated 05/13/2018.Plan of Correction: I will notify a the staff person that they must get a new TB test by March 5TH . She will remain off the schedule until it is taken care of.
Standard #: 22VAC40-185-550-M Description: Based on review of injury records, Licensee did not ensure all required information was included in injur .
Evidence: Injury report for child #1 was incomplete. The injury record for Child #1 and Child #2 did not contain date and time parent was notified.Plan of Correction: We will reiterate with staff the proper way to refill out these reports. We will also have the assistant director and director will check them when they are filled out.
