This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on February 10, 2021 and concluded on February 11, 2021. The director was contacted by telephone to initiate the inspection. There were 150 children present and 30 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed five children?s records and five staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-185-130-B Description: Based on a record review, the center did not ensure documentation was obtained for additional immunizations once every six months for children under the age of two years.
Evidence:
1. The record for child #3 (DOB 6-25-19) contained documentation of immunizations dated 8/29/19 and 9/10/20.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 63.2(17)-1720.1-B-4 Description: Based on a review of staff records, the center failed to obtain the required out of state background checks for an employee who lived outside of Virginia within the last five years.
Evidence:
1. Staff #3 began employment on 11/09/2020. This staff person resided in one state other than Virginia within the past 5 years.
2. Administrator #1 verified that the Child Abuse and Neglect Registry, Criminal History, and the Sex offender registry had not been submitted within the required time frame.Plan of Correction: Not available online. Contact Inspector for more information.
