Technical Assistance:

Only staff members with fully completed background checks can be left alone with children and/or counted in required staff:child ratios. It is highly recommended that all evacuation drills be documented on the Division's model form.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 3/1/2021 and concluded on 3/2/2021. The owner was contacted by telephone to initiate the inspection. There were 15 children present and 4 staff. The inspector emailed the owner a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #:	22VAC40-185-160-A
Description:	Based on a review of staff health records, it was determined that each staff member did not submit documentation of a negative tuberculosis screening no later than 21 days after employment. Evidence: 1 - Staff A began employment on 10/29/2020. As of the date of this inspection on 3/1/2021 the staff person has not submitted documentation of a negative tuberculosis screening. 2 - Staff B began employment on 8/30/2020. As of the date of this inspection on 3/1/2020 the staff person has not submitted documentation of a negative tuberculosis screening.
Plan of Correction:	Both have scheduled their TB tests for March 15. They were nervous to get a TB test earlier, due to COVID.
Standard #:	22VAC40-185-260-A
Description:	Based on a review of administrative documents, it was determined that the center was not able to provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: The most recent fire inspection was completed on 11/30/2018. This fire permit shows an expiration date of 10/1/2019.
Plan of Correction:	I will contact the county for an inspection.
Standard #:	22VAC40-191-60-C-2
Description:	Based on a review of staff background checks, it was determined that one staff person does not have a central registry finding within 30 days of employment or volunteer service. Evidence: Staff B began employment on 8/30/2020. As of the date of this inspection this no documentation of a completed central registry background search.
Plan of Correction:	She will get the form notarized today and I will submit the request tomorrow. I will send a copy of the request to the Inspector within 10 days as required.
