This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 03/05/2021 and concluded on 03/10/2021. The facility representative was contacted by telephone to initiate the inspection. There were 20 children present and 4 staff. The inspector emailed the facility representative a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard #: 22VAC40-185-60-A Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #1 did not contain work phone number of the parent. The record for Child #2 did not contain work phone number of the parent who has custody and the addresses of two designated people to call in an emergency if a parent cannot be reached.Plan of Correction: The center shall contact the parents to provide the information.
