Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on March 1, 2021 and concluded on March 3, 2021. The director was contacted by telephone to initiate the inspection. There were 28 children present with eight staff. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed three children's records and seven staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Violations:

Standard #:	22VAC40-185-70-A
Description:	Based on a review of staff records, the center failed to ensure staff references were completed before the first day of employment. Evidence: 1. Staff #1's date of hire was 09/28/2020. One of the references was completed and dated 10/01/2020, and the other reference was completed and dated 10/08/2020. 2. Staff #2's date of hire was 12/07/2020. There were no documentation of reference checks being completed. 3. Staff #3's date of hire was 02/03/2021. There was no documentation of reference checks being completed. 4. Staff verified the dates the reference checks were completed for Staff #1, and verified that there had not been reference checks completed on Staff #2 or Staff #3.
Plan of Correction:	All staff records will be viewed by the director and financial officer prior to hiring. The director will also attend the next VDSS Phase 2 CDC Training that is offered.
Standard #:	22VAC40-185-510-D
Description:	Based on a review of medications, the center did not ensure that medication authorizations were available to staff during the entire time it was effective. Evidence: 1. Child 1M has a prescription emergency medication at the center. The medication authorization form was reviewed and there was not a parent signature on the form authorizing the medication. 2. There was a physician signature dated 09/22/2020. 3. Staff verified that there was not a parent signature authorizing the medication if needed.
Plan of Correction:	Administration contact the parent and obtained the required signature.
Standard #:	63.2(17)-1720.1-B-4
Description:	Based on review of staff records, the center failed to ensure that an out of state sex offender registry check, and an out of state criminal history record check had been completed within the required time frame. Evidence: 1. Staff #1's date of hire was 09/28/2020. On the sworn disclosure statement dated 9/18/2020, Staff #1 indicated they had lived outside of Virginia within the past five years. 2. There was no documentation of an out of state sex offender registry check being completed. The requirement states that any new employee hired after 07/01/2020 a sex offender registry check was required by 10/01/2020. 3. The state indicated was a state that does not report to the national fingerprint, therefore an out of state criminal history check was required. There was no documentation of the out of state criminal history record check being completed. The requirement states that for any new employee hired after 07/01/2020 an out of state criminal history check was required by 10/01/2020. 4. Staff verified that an out of state sex offender registry check, and an out of state criminal history check were not completed on Staff #1.
