Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 03/09/2021 and concluded on 03/11/2021. The in-charge person was contacted by telephone to initiate the inspection. There were 69 children present and 22 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed four children?s records and four staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable
standards or law and violations were documented on the violation notice issued to the facility.

Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return it to me within 5 business days from the date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for
Child Care programs.

Violations:
Standard #: 22VAC40-185-510-L
Description: Based on record review and interview, the center did not keep a record of medication given to one child with all the required information.

Evidence: 1) The medication administration documentation reviewed for Child #5 did not include documentation of any adverse reactions or any medication error for two medications that were administered in the month of February 2021. The medications are administered daily whenever the child is in care. 2) During interview, a member of management acknowledged the medication administration documentation did not include documentation on any adverse reactions or any medication errors.

Plan of Correction: Per the Center: "As of 3/11/21, Westminster Canterbury CDC will no longer log medication information electronically in Tadpoles. On 3/11/21, WCR CDC Staff were notified of violation and instructed to log all required medication information on the MAT Log of Medication Administration form (Handout 3.2), which includes adverse reactions/side effects and medication errors. Teachers will notify Administrative Assistant when medication log has been completed and Administrative Assistant will scan logs into a monthly file."

Description: Based on a review of seven written records of children's serious and minor injuries, the center did not ensure four written records contained the required information.

Evidence: 1) An incident report for a minor injury that occurred on 02/05/21 was reviewed for Child #7. The incident report did not contain documentation on how the child's parent was notified.

2) An incident report for a minor injury that occurred on 02/08/21 was reviewed for Child #8. The incident report did not contain documentation on how the child's parent was notified.

3) An incident report for a minor injury that occurred on 02/15/21 was reviewed for Child #10. The incident report did not contain documentation on how the child's parent was notified.

4) An incident report for a minor injury that occurred on 02/04/21 was reviewed for Child #11. The incident report did not contain documentation any future action to prevent recurrence of the injury and the time parents were notified.

Plan of Correction: Per the Center: "On 3/11/21, WCR CDC Administrative Assistant edited the Accident Report by adding ?Method of Notification.? 0n 3/11/21, WCR CDC Staff were informed of the violation and advised to include all required information onto the Accident Report. Once the Accident Report has been completed, the teachers will submit it to the Administrative Assistant, to review and scan into a monthly file."
