Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection with the local agency was initiated on 2/23/21 and concluded on 3/17/21. A self-reported incident was received by the department on 2/9/21 regarding allegations the care of the children. The director was contacted by telephone to conduct the investigation. The inspector did a virtual tour of the center and conducted interviews of staff. The evidence gathered during the investigation resulted in violations of the standards that are listed on the violation report.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on a review of two staff records, it was determined that the facility did not ensure that at least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening.

Evidence:
1. The record for staff #2, contained documentation of TB screening that was dated 1/22/18.
2. Staff #1 (Program Director), reviewed the record for staff #2 and confirmed that an updated TB screening had not been received.

Plan of Correction: The facility responded: Staff member will be sent for a TB test on Friday, 3/26 with results expected no later than Tuesday 3/30.

Standard #: 22VAC40-185-40-E
Description: Based on a review of the facility's policies and interviews, it was determined that the licensee did not ensure that the facility follows it's own policies and procedures for supervision of children in care.

Evidence:
1. The facility's policy states that children will be placed in counselor groups. Each counselor group is assigned a specific counselor,
Counselors will remain with their group throughout the session to help maintain a routine that the children are familiar with.
2. On February 9, 2021 child #1 was seen on video exiting the facility alone and unsupervised through the front door of the facility and getting in her mother's car.
2. Staff #2 was unaware that child #1 had left the facility until child #1's step father called the facility to say he was picking her up.
3. Staff #1 (Program Director) confirmed that child #1 was assigned to staff #2's group, and that they did not comply with their own policy for being accountable to the children in care are at all times.

Plan of Correction: The facility responded: Review Supervision Policy with staff. Ensure staff have their group rosters & are checking rosters regularly. The Program Director will also review the attendance check policy with all staff.

Standard #: 22VAC40-185-340-F
Description: Based on interviews and a review of documentation, it was determined that the licensee did not ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff.

Evidence:
1. On February 9, 2021 at approximately 2:06pm child #1 was seen on video exiting the facility alone and unsupervised through the front door of the facility and getting in her mother's car.
2. Staff #1 (Program Director) confirmed that child #1 was not always within actual sight and sound supervision of staff.

Plan of Correction: The facility responded: Review Supervision Policy with all staff. Work one on one with staff involved to ensure an understanding of the policy and work on strategies to follow policy.
