Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 3/24/21 and concluded on 3/25/21. The director was contacted by telephone to initiate the inspection. There were 21 children present and 5 staff present. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on a review of two staff records, it was determined that the facility did not ensure that at least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening.

Evidence:
1. The record for staff #1, contained documentation of TB screening that was dated 3/1/19.
2. Staff #1 (Program Director), reviewed his record. and confirmed that he had not completed an updated TB screening.

Plan of Correction: The facility responded: I will complete an updated TB screening. All current staff will complete an updated TB screening every two years.

Standard #: 22VAC40-185-60-A
Description: Based on a review of three children's records and interview, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each child enrolled that contains all required information.

Evidence
1. The record for child #3 did not contain documentation for viewing the child's proof of identity.
2. Staff #1 (Site Director) confirmed that the record for child #3 did not contain all of the required information.

Plan of Correction: The facility responded: The parents of child #3 were contacted to provide the proof of birth documentation. All newly enrolled children will have their records reviewed at enrollment to ensure they are complete.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on a review of two staff records, it was determined that the provider did not ensure that the center obtain a copy of the results of a criminal history record information check and sex offender registry check from any state in which the applicant has resided in the preceding five years.

Evidence:
1. The record for staff #2 (date of hire 3/2/210) did not contain documentation of a completed criminal history record information check and sex offender registry check from the state of Washington. The sworn statement or affirmation located in the record for staff #2 indicated that staff #2 lived in the state of Washington within the last 5 years.
2. Staff #1 (Program Director) reviewed the record for staff #2, and confirmed that the criminal history record information check and sex offender registry check from the state of Washington had not been completed prior to employment

Plan of Correction: The provider responded: The request for the criminal history record information check and sex offender registry check Washington for staff #2 will be sent. All new employees that have lived outside of the state of Virginia will not start until the criminal history record information check and sex offender registry check from the state they previously resided in has been received. We will forward the results to our Licensing Inspector when it is received.
