This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 03/25/2021 and concluded on 03/25/2021. The director was contacted by telephone to initiate the inspection and the inspection was completed virtually via FaceTime. There were 89 children present and 21 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 5 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-185-60-A Description: Based on record review, 1 of 5 children's records did not contain all required information.
Evidence:
1. The record for Child #1 did not contain the work phone number for Child #1's father.Plan of Correction: We will ask the parent to fill out the information on the form.
Standard #: 22VAC40-185-550-M Description: Based on record review, 1 of 4 injury reports did not contain all required information.
Evidence:
1. An injury report dated 3/19/2021 in the record for Child #2 did not contain documentation of future action to prevent recurrence of the injury.Plan of Correction: We will write a future action to prevent the injury today and do a staff training in April.
Standard #: 63.2(17)-1720.1-B-4 Description: Based on record review, the center did not obtain within 30 days of employment, a search of the child abuse and neglect registry from any state in which the individual has resided in the preceding five years for one staff.
Evidence:
1. Staff #1 (hire date 09/08/2020) has resided outside of the state of Virginia in the past 5 years.
2. Documentation in the record for Staff #1 indicated a child abuse and neglect search request for Washington DC was mailed on 9/10/2020; however results of the search were not obtained until 12/10/2020.
3. The record for Staff #1 did not contain documentation that the center followed up on the search of the child abuse and neglect registry for Washington DC, 45 days after the request to search.Plan of Correction: Results of the search were obtained and placed in the record for Staff #1.
