Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was conducted on 03/02/2021 and 03/04/2021 in response to a self-reported indecent involving an injury to a child that occurred on 03/01/2021.

Based on the information gathered which included children and staff interviews, a review of records and virtual inspection of the I-Zone classroom violations were found in the areas of staffing and supervision and the physical plant.

The violations are listed on the violation notice issued to the facility and were reviewed with the program director at the exit interview.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on interview and facility review, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition.

Evidence:
1. Child 1, age 6 years, received an injury requiring medical attention when he jumped from a stack of chairs in the I-Zone classroom on 03/01/2021 at 2:00pm.
2. Staff 1 and staff 2 stated there were 2 stacks of classroom chairs, stacked 3 chairs high, in the I-Zone classroom.
a. Staff 2 stated that he was aware these chairs were stacked up in the I-Zone classroom but they had not been removed as "We didn't have the space."
3. Staff 1 stated she verbally instructed all of the children to stay off of the chairs when they entered the I-Zone classroom because she was aware that the children liked to climb up on them.
a. Staff 1 stated she had to remove child 1 from the stack of chairs once prior to the fall that caused his injury.
4. Child 1 and child 2 confirmed that there were two stacks of chairs placed against the wall in the I-Zone classroom.
a. Both children stated that the stacks of chairs were always there.
b. Child 1 stated the chairs were stacked up 9 or 10 high on 03/01/2021.
c. Child 2 stated the chairs were stacked 5 high on 03/01/2021.

Plan of Correction: The program director will be responsible for the day to day management of the program including all aspects of supervision of children and building maintenance. The PD has conducted a training on "Designing effective indoor play areas and developing and implementing indoor interest areas." New procedure was implemented 03/02/2021, where staff were fully trained on this new procedure.
*The PD and staff walked all programs areas and discussed ways to improve them after the trainings were complete.
*The PD removed all stacks of chairs from every program area as of 03/02/2021.
*The PD has implemented a protocol for staff to communicate any potential hazards before children report to any program area.
*The PD has developed a google form checklist to address any potential hazards.

Standard #: 22VAC40-185-340-A
Description: Based on staff interviews, the center failed to ensure that when staff are supervising children, they shall always ensure their care, protection, and guidance.

Evidence:
1. Staff 1 stated that child 1 was playing on the stack of chairs that had been placed up against the wall in the I-Zone classroom on the afternoon of 03/01/2021.
a. Staff 1 indicated she had to verbally instruct child 1 several times to come down from the chairs.
b. Staff 1 stated she redirected child 1 to an art activity.
2. Staff 1 confirmed that she did not remove the stack of chairs even though child 1 had been climbing on the chairs and that there had been 3 or 4 instances, in the past, of children climbing on the chairs.
3. Approximately 5 minutes later, child 1 climbed back up on the stack of chairs and fell on the floor resulting in an injury to his right arm which required medical attention.

Plan of Correction: The PD will be responsible for the day to day management of the program including all aspects of supervision of children and building maintenance. The PD has conducted a training on the proper procedures for conducting a name to face check to ensure proper accountability of all children in care throughout the day. The PD also conducted a training on staff placement to ensure proper sight and sound of the children throughout the day. This updated procedure was implemented on 03/09/2021 where staff were fully trained on this new procedure.
Staff Placement & Supervision Training:
The PD has reviewed the best practices of staff placement and where staff should be in each program area. Staff should never have their back to any children and always have eyes on the group. This included outside field, IZone, enrichment area, and gym. Also discussed having focus supervision with each group and being vigilant. This training included a focus on closing off areas of usage if necessary. Having at least 2 staff at all times when there are more than 15 children in a program area. No group of children should be left alone if the group only has one counselor. Conduct name-to-face checks to ensure proper headcount every 5-7 minutes, every child accounted for. The PD and the staff also walked each program area and discussed where the staff should be positioned in that space. This staff training was conducted 03/07/2021 at the Hilltop YMCA.
Name To Face Head Count Procedure:
1. Make sure you have the correct roster and that it is up-to-date.
2. Have all children in your care in a designated area.
3. Check off each child by having them come to you or moving to another area.
4. Once you have gone through every child, make sure counts of children and actual children match.
5. Do this every time you go in transition.
6. As children leave, indicate that on the roster.
7. Do a complete sweep of area before leaving.

Standard #: 22VAC40-185-340-F
Description: Based on staff and children interviews, the center failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff.

Evidence:
At approximately 2:00 pm on 03/01/2021, child 1, age 6 years, jumped from a stack of chairs resulting in a fracture to the right arm above the elbow.
2. Staff 1 stated she did not see the incident as she was preparing an art activity. "I heard a sound and screaming.....I turned around and saw child 1 on the floor crying and holding his arm."
a. Staff 1 stated that she asked child 2 to describe what had happened because she did not know what lead up to child 1 falling on the floor.
3. Child 2 verified that he had been asked to describe what had occurred by staff 1.
a. Child 2 stated that staff 1 did not see the incident because she was standing in the doorway with her back to the children in the classroom. "She was in the doorway watching children wash hands in the bathroom."
