Comments:
This inspection was conducted by licensing staff using an alternate protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 04/23/2021 and concluded on 04/26/2021. The director was contacted by telephone to initiate the inspection. There were 20 children present and 5 staff. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed 1 staff record, 5 board member records and 2 children's records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

The licensing inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Violations:
Standard #: 22VAC40-185-500-A
Description: Based on interview, the center failed to ensure that children's hands shall be washed with soap and running water or disposable wipes after eating snacks.

Evidence:
1. Staff reported during the virtual inspection on 04/26/2021 that children used hand sanitizer to clean hands after eating afternoon snack.

Plan of Correction: Each cohort will ensure that children's hands shall be washed with soap and running water or use disposable wipes.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on record review and interview the licensee failed to ensure that every agent of a child day center shall undergo a background check prior to serving as a volunteer or agent.

Evidence:
1. Board member 1 lacks a completed sworn statement or affirmation, child protective services central registry check and a fingerprint background check.
a. The SCC Website indicates that board member 1 was added to the board of the corporation on 02/17/2021.
2. The background checks for board member 2 were obtained after appointment to the board of the corporation which took place on 02/17/2021 according to the SCC website.
a. The sworn statement or affirmation was completed on 03/31/2021.
b. The fingerprint background check was completed on 04/12/2021.
c. The central registry check was completed on 04/12/2021.
2. The program director confirmed that background checks were not available for board member 1 and that background checks for board member 2 had been obtained after appointment to the board.

Plan of Correction: Background checks for Board Members will take place within 10 days of them coming on the Board of Trustees.

BOT 1: Provide Sworn Statement. CPS, CRR and fingerprint background check when member receives documents after requesting name change from social security.

2c. The business office will resubmit the CRS Search for Board Member 2 on May 4, 2021 because CRS search was returned with an incorrect name.
