Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 4/23/2021 and concluded on 4/23/2021. The director was contacted by telephone to initiate the inspection. There were 31 children present and 7 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Standard #:	22VAC40-185-60-A
Description:	Based on review of three children's files, the facility failed to have all required information in each file. Evidence: the two emergency contacts listed for Child 3 did not have the addresses listed as required.
Plan of Correction:	Center director will request the missing information from the parents.
Standard #:	22VAC40-185-70-A
Description:	Repeat violation: Based on review of two staff files, the facility failed to have all required documents in each staff file. Evidence: Staff B did not have documentation of two references before employment as required. Staff B (date of hire 4/19/2021) did not have documentation of orientation by the end of the first day of employment.
Plan of Correction:	Center director will get the missing information for the file.

Description:	Repeat violation: Based on review of three staff files, the facility failed to have a signed sworn disclosure statement for each staff prior to the first day of employment. Evidence: Staff B (date of hire 4/19/2021) and Staff C (date of hire 12/2019) did not have documentation of a sworn disclosure statement.
Plan of Correction:	Center director will get a sworn disclosure statement from Staff B.

Description:	Repeat violation: Based on review of three staff files, the facility failed to have documentation of a search of the central registry by the 30th day of employment. Evidence: Staff A (date of hire 6/15/2020) and Staff C (date of hire 12/2019) did not have documentation of a search of the central registry (CPS check).
Plan of Correction:	Center director will get a search of the central registry for each new staff.

Description:	Based on review of three staff files, the facility failed to obtain fingerprint results for each staff prior to hire. Evidence: Staff C (date of hire 12/2019) did not have documentation of fingerprint results.
Plan of Correction:	All staff will obtain fingerprint checks prior to hire.
