Standard #: 22VAC40-185-550-M Description: Based on record review and interview, the center failed to maintain a written record of a child's injury that included all the required components.
Evidence:
1. The injury report dated 10/16/20 only contained one signature and did not document the method of notification.
2. The director stated the parent was notified at pickup and she is unaware of why the report was not document and why the parent was not asked to sign the report.Plan of Correction: A staff training was conducted last week about mandated reporting and incident reports to give everyone a refresher.
