This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on May 10, 2021 and concluded on May 13, 2021. The director/owner was contacted by telephone to initiate the inspection. There were 36 children present and 8 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-185-60-A Description: Based on review of children's record and interview, the center did not ensure to obtain all required information.
Evidence: In Child#3's record (start date:4/26/21), the name, address, number of another contact person to call in an emergency if a parent cannot be reached was not available for review. Interview with staff confirm this information was not available during inspection.Plan of Correction: Will obtain the information for an emergency contact today. ( will contact parent)
