Standard #: 22VAC40-665-580-H Description: Based on a review of records on May 7, 2021, the vendor did not ensure that each staff complete the Virginia Annual Health and Safety Update Training by March 1, 2020, and annually thereafter.
Evidence: 1. The record of Staff #2 (DOH 8/12/16) contained documentation of the 3-hour annual health and safety update training dated 11/16/20.
2. The record of Staff #3 (DOH 3/4/13) contained documentation of the 3-hour annual health and safety update training dated 11/10/20.Plan of Correction: Check the pre-service training each year by February by VP.
