Standard #: 63.2(17)-1720.1-B-4 Description: Based on record review and interview it was determined the center did not obtain a copy of the results of a sex offender registry check and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years prior to the first day of hire or service.
Evidence:
1. The record for staff #1(hire 8/21/19) did not contain documentation of a search of the criminal history record information check, child abuse and neglect registry and sex offender registry check for Pennsylvania. Staff #1 indicated on their sworn statement that they had lived in Pennsylvania within the last 5 years.
2. Staff #2 confirmed that there were no criminal history record information check, child abuse and neglect registry, and sex offender registry check results from Pennsylvania for staff #1 available during this inspection.Plan of Correction: Director will resubmit the Pennsylvania background check requests for staff #1.
