Evidence: The most recent central registry check on file for applicant A was dated 10/21/2015 and the most recent sworn statement form on file for applicant A was dated 09/10/2015. Plan of Correction: The applicant will complete the central registry check and sworn statement and submit the results to licensing. Applicant background checks are completed at the business entity's location in Arizona. Applicant is not involved in supervision of children within this facility.
