Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 6/21/21 and concluded on 6/22/21. The center director was contacted by telephone to initiate the inspection. There were 80 children present and 19 staff. The inspector emailed the administrator a list of items required to complete the inspection. The Inspector reviewed 4 children's records and 4 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review and interview, the center did not ensure that each child's record shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.

Evidence:
1. The record for child 1 (date of enrollment: 3/1/21) does not contain documentation of a physical examination.
2. The center director confirmed that the record for child 1 is lacking documentation of a physical examination.

Plan of Correction: The center responded with the following: The parent of child 1 has been contacted and a copy of the most recent physical was requested.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and interview, the center did not ensure that all staff shall have a central registry finding within 30 days of employment.

Evidence:
1. The record for staff 1 (date of hire: 5/19/21) does not contain documentation of a central registry finding within 30 days of employment.
2. The center director confirmed that the record for staff 1 is lacking documentation of a central registry finding.

Plan of Correction: The center responded with the following: The central registry search requests for staff 1 was mailed upon hire; however, the results have not been received. Administration will follow up on the request and a new request will be sent if the results can not be located.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on record review and interview, the center did not ensure that a copy of the results of a sex offender registry check is obtained from any state in which the individual has resided in the preceding five years.

Evidence:
1. Staff 2 has a hire date of 6/16/21. Staff 2 indicated on her sworn statement or affirmation that she has resided in the state of Florida within the past five years. The results of a sex offender registry check were not available for staff 2 from the state of Florida.
2. The center director confirmed that the Florida sex offender registry check was not completed for staff 2.

Plan of Correction: The center responded with the following: The Florida sex offender registry check for staff 2 was completed during the inspection.
