Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on conducted on July 16, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were seventy-four children present, ranging in ages from birth to school age, with nineteen staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review and interview, facility did not ensure 1 of 4 children's records has documentation of required information.

Evidence: The record for child #1 (start date 5/28/2019) did not have complete addresses for the two emergency contacts. Director confirmed information was missing from child's record.

Plan of Correction: We got the information from parents.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on observation and interview, facility did not ensure there is required out-of-state background checks when an employee has lived outside the state of Virginia within the past five years.

Evidence: The record for staff #1 (start state 2/3/2021) had documentation of living outside the state of Virginia within the past five years and there was not a completed sex offender check and child abuse/neglect registry check for that state. Director confirmed these background checks were not conducted.

Plan of Correction: I will do both checks asap.
