Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.

A renewal inspection was initiated on 8/18/2021 and concluded on 8/19/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 28 children present, ranging in ages from 3 months to 4 years, with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records, 7 staff records, and 5 medications were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-510-A
Description: Based on medication review and staff interview, it was determined that not all children with medication on site had documentation of a parental authorization form.

Evidence: Child #6 has Benadryl on site and did not have signed parental authorization form on file for the medication.

Plan of Correction: Child now has signed parental authorization.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on record review and staff interview, the center did not obtain the results of a sex offender name search from all states in which staff have lived within the last 5 years.

Evidence: Staff #5 (DOH: 5/4/21) and Staff #7 (DOH: 2/15/21) have both resided in New York within the last 5 years and didn't have sex offender name search results on file.

Plan of Correction: Both staff had the NY Central Registry Background Check on file. On 8/20/21, Director faxed staff info to the NY Staff Division of Criminal Justice Services. On 8/23/21, Director spoke with the NY Staff Division and verified that neither staff were on the sex offender registry. Written results will be received within the next three weeks.
