Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable). A subsidy inspection was initiated on 8/17/21 and concluded on 8/18/21. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-665-540-A
Description: Based on a review of 16 staff records, it was determined that the vendor did not ensure that staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence:
1. The record for staff #1 (date of hire 6/15/21) contained documentation of a negative TB screening that was dated 6/18/21.
2. The record for staff #4 (date of hire 6/16/21) contained documentation of a negative TB screening that was dated 6/20/21.
3. The record for staff #12 (date of hire 6/4/21) contained documentation of a negative TB screening that was dated 6/18/21.
4. The record for staff #14 (date of hire 6/9/21) contained documentation of a negative TB screening that was dated 6/21/21.
5. Staff #17 (Program Administrator) confirmed that the staff listed above did not complete a TB screening prior to employment

Plan of Correction: The facility responded: The Child Care facility at Taylor Bend Family YMCA will ensure that each staff member will submit documentation of a negative tuberculosis screening. Documentation of a negative TB screening has to be completed prior to the staff?s start date and must be less than 30 days old.

Standard #: 22VAC40-665-580-B
Description: Based on a review of 16 staff records, it was determined that the vendor did not ensure that within the first 90 days of employment or subsidy vendor approval all staff who work directly with children shall complete Virginia Preservice Training for Child Care Staff, which shall include training on the following topics and training modules.

Evidence:
1. The record for staff #2 (date of hire 3/11/21) did not include documentation for the Virginia Preservice Training for Child Care Staff.
2. Staff #17 (Program Administrator) confirmed that staff #2 had not completed the required training within 90 days of hire.

Plan of Correction: The facility responded: The Child Care facility at Taylor Bend Family YMCA will ensure that staff records will contain all required documentation for each staff (orientation, pre-service training 10 hour and 3 hour update within 90 days)
