The following was discussed with the assistant director:
The slide on the property does not have resilient surfacing. The assistant director stated the slide is no longer used. It was recommended, the center put caution tape around the piece of equipment to ensure it is not used.

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated on 08/20/21 and concluded on 08/30/21. The in-charge person was contacted by telephone to initiate the inspection. There were 17 children present and 4 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
