Standard #: 22VAC40-185-60-A Description: Based upon review of two children's records, the facility has not ensured that each child's record includes the names, addresses and phone numbers of two designated people to call in an emergency if a parent cannot be reached.

The record provided for child 2 included the name, address and telephone number for only one emergency contact person.Plan of Correction: The facility responded with the following:
A second emergency contact person was added to the file of child 2. In the future we will ensure that all children's records have at least two emergency contact person's other than the parents.
