Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A subsidy health and safety (SHSI) inspection was initiated on August 27, 2021 and concluded on August 30, 2021. There were 160 children present with 18 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 22VAC40-665-580-B
Description: Based on a review of records and interview, the provider did not ensure within the first 90 days of employment of service all caregivers complete the Virginia Pre-Service Training for childcare staff, sponsored by the Department of Social Services.
Evidence: 1. The record of Staff #1 (DOH 9/26/16) did not contain documentation of Pre-Service training.
2. Administration acknowledged the training was not complete.

Plan of Correction: For staff who are continuing on receive CPR/FA and Preservice training.

Standard #: 22VAC40-665-580-E-1
Description: Based on a review of records and interview, the provider did not ensure to obtain current certification, appropriate to the ages of children in care, in cardiopulmonary resuscitation, within 90 days of employment for all staff.
Evidence: 1. The records of Staff #2 (DOH 6/14/18), Staff #3 (DOH 5/19/21), and Staff #4 (DOH 5/25/21) did not contain documentation of cardiopulmonary resuscitation certification.
2. Administration acknowledged the cardiopulmonary resuscitation certification was not complete.

Plan of Correction: For the staff who are continuing on, receive CPR/FA and Preservice training.

Description: Based on a review of records and interview, the provider did not ensure to obtain current certification, appropriate to the ages of children in care, in first aid within 90 days of employment for all staff.
Evidence: 1. The records of Staff #2 (DOH 6/14/18), Staff #3 (DOH 5/19/21), and Staff #4 (DOH 5/25/21) did not contain documentation of first aid certification.
2. Administration acknowledged the first aid certification was not complete.

Plan of Correction: For the staff who are continuing on, receive CPR/FA and Preservice training.

Standard #: 22VAC40-665-580-H
Description: Based on a review of records and interview, the vendor did not ensure that each staff complete annually the Virginia Annual Health and Safety Update Course sponsored by the Department of Social Services as required.
Evidence: 1.The records of Staff #1 (DOH 9/28/16), Staff #2 (DOH 6/14/18), and Staff #5 (DOH 8/28/20) did not contain documentation of the 3-hour annual health and safety update training.
2. Administration acknowledged the training was not complete.

Plan of Correction: For staff who are continuing on, receive CPR/FA and Preservice training.
