This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.
A monitoring inspection was initiated on 09/03/2021 and concluded on 09/08/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 9 children, ages 13 months- 3 years, and 3 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, emergencies and nutrition. A total of 2 children?s records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 22VAC40-185-140-A Description: Based on record review and discussion with staff, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.
Evidence: The record for Child 1 had a documented start date of 05/10/2021. There was not documentation of a physical examination in the record for Child 1 during the 09/08/2021 inspection.Plan of Correction: Will request the information from the parent.
