A monitoring inspection was initiated on 09/08/2021 and concluded on 09/15/2021. The director was contacted by telephone to initiate the inspection. There were 18 children present and 2 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-185-130-A Description: Based on review of two children's records, the center failed to ensure that all immunizations were obtained and in the child's records at the facility.
Evidence:
1. Child # 1 started the afterschool program on 8/11/2021 and did not have a copy of the immunizations.Plan of Correction: The director stated that they are working with the parent, the parent will submit the missing paperwork ASAP.
Standard #: 22VAC40-185-140-A Description: Based on review of two children's records, the center failed to ensure that all physical exams were obtained and in the child's records at the facility.
Evidence:
1. Child # 1 started the afterschool program on 8/11/2021 and did not have a copy of the physical exam with in the 30 days of enrollment.Plan of Correction: The director stated that they are working with the parent, the parent will submit the missing paperwork ASAP.
Standard #: 22VAC40-185-160-C Description: Based on review of two staff's records, the center failed to ensure that all staff had repeat TB screening.
Evidence:
1. Staff # 1 most recent TB was 9/22/2018, the TB is required to be updated every two years.Plan of Correction: The director sent the staff person today to obtain a new TB screening. The director will monitor all dates.
