This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).
A monitoring inspection was initiated on Tuesday, September 28, 2021 and concluded on Wednesday, September 29, 2021.The provider was contacted by telephone and a virtual inspection was conducted. There were 9 children present with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed. Information gathered during the inspection determined non- compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22VAC40-185-60-A Description: Based on review of children's record and staff interview, the center did not ensure to obtain all required information.
Evidence: In Child#2's record (start date 09/13/21) there was no documentation of viewing proof of the child's identity and age available for review during the inspection. Interview with staff confirmed the documentation was not provided for review.Plan of Correction: The center's family advocate will reach out to the family to obtain birth certificate or alternative documents by October 15, 2021.
