Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on September 21, 2021 and concluded on September 23, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 9 children present, ranging in ages from 3 yeas to 4 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 22VAC40-185-200-A
Description: Based on a review of staff records and interview, the center failed to have a qualified director or qualified back-up program director who meets one of the director qualifications who is regularly on site at least 50% of the center's hours of operation.
Evidence: 1. Administration identified Staff #2 as the program director. The record of Staff #2 did not contain documentation of program director qualifications.
2. Staff confirmed there was not a qualified program director on site at least 50% of the time.

Plan of Correction: The site manager will spend 50% of time at the center to meet the licensing requirement for program director. The center leader will seek further TA to determine proof of qualifications for staff 2.

Standard #: 22VAC40-185-340-D
Description: Based on a review of staff records and interview, the center failed to have at least one staff member who meets qualifications of a program leader regularly present in each grouping of children.
Evidence: Staff #1 and Staff #2 are regularly scheduled as the only staff present in the head start classroom but neither are qualified to be a program leader.

Plan of Correction: Staff 1 and 2 both have CDA with over 120 hours of education credits. Both staff have programmatic experience of 3 years and plus. The center will see TA to determine how to further prove qualifications for program leadership.
