Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.

A subsidy health and safety inspection was initiated on 10/04/2021, a virtual inspection of the program was conducted on 10/12/2021 and the inspection concluded with a record review on 10/19/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 41 children, ages 5 years-11 years, and 5 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, emergencies and nutrition. A total of 2 children?s records and 2 staff records were reviewed. A licensing inspection was also conducted on this date. Both inspections combined cover health and safety regulations. Only subsidy requirements not cover in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Description: Based on record review and discussion with staff, the vendor failed to obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: The record for Child 1 contained documentation of a diagnosed food allergy. The record for Child 1 did not contain written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: The child will be removed from the program until the allergy care plan is received.
