?8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes

Comments:

An unannounced subsidy health and safety inspection (SHSI) was initiated and concluded on October 27, 2022. The licensing inspector was onsite from approximately 9:30am until 11:00am. There were zero children in care and one caregiver present. A monitoring inspection was also conducted on this date. The information gathered during the inspection determined non-compliances with subsidy regulations, and violations were documented on the violation notice issued to the facility. Standard #:	8VAC20-790-190-B
Description:	Based on review of two children?s records and interview, the vendor did not ensure two children?s records contained the required information. Evidence: The records for Child #1 and Child #2 did not contain a name, address, and telephone number of at least one person designated by the parent to contact in case of an emergency if the parent cannot be reached. The vendor confirmed this information during the inspection.
Plan of Correction:	I will contact the parent and request the emergency contact information.
Standard #:	8VAC20-790-210-B
Description:	Based on an interview, the vendor did not ensure one caregivers obtained a tuberculosis (TB) screening at least every two years from the date of the initial screening, or more frequently if recommended by a physician. Evidence: Caregiver #1 stated her TB screening was overdue and she would be obtaining it soon. The previous TB screening could not be located during the inspection.
Plan of Correction:	I will schedule an important to update my TB screening and send the results to the licensing inspector.
Standard #:	8VAC20-790-230-A
Description:	Based on review of two children?s records and interview, the vendor did not obtain documentation that the child has been immunized according to the requirements of subsection A of ? 32.1-46 of the Code of Virginia and applicable State Board of Health regulations before children began attending the family day home. Evidence: The records for Child #1 and Child #2 (first date of attending 10/19/2019) did not contain documentation of immunizations. Vendor #1 stated she needed to ask the family for a copy of the immunization records.
Plan of Correction:	I will request a copy of the shot records from the parent and add it to the record.
Standard #:	8VAC20-790-250-F
Description:	Based on review of one caregiver?s record and interview, the vendor did not ensure one caregiver that worked directly with children obtained at least 16 hours of training annually, to include the department's health and safety update course. Evidence: The record for Caregiver #1 did not contain annual training hours. Caregiver #1 stated the required 16 hours of annual training was not completed.
Plan of Correction:	I will look into additional training and make sure to complete the required training hours for next year.
Standard #:	8VAC20-790-420-A
Description:	Based on documentation review and interview, the vendor did not have a written emergency preparedness plan that addresses caregiver responsibility and home readiness with respect to emergency evacuation, relocation, lockdown, and shelter-in-place procedures. The plan shall address the most likely to occur emergency scenarios, including fire, severe storms, flooding, tornadoes, loss of utilities, earthquakes, intruders, violence on or near the premises, chemical spills, and facility damage or other situations that may require evacuation, lockdown, or shelter-in-place.
Plan of Correction:	I will print off a new template off and complete the information.
