This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A renewal inspection was initiated on November 5, 2021 and concluded on November 15, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 27 children present, ranging in ages from two years to five years of age, with six staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and seven staff records were reviewed.
New requirements became effective October 13, 2021. The facility has not yet fully complied with orientation requirements. The facility will review the new requirements and work with their assigned licensing inspector to ensure future compliance.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-191-40-D-1-A Description: Based on a review of board member background checks, the center failed to obtain a completed sworn disclosure statement within 30 days of beginning service.
Evidence:
1. There was no documentation of a completed sworn disclosure statement on file for Agent #2. Agent #2 began service on the board in October 2020.
2. Staff verified that there was not a completed sworn disclosure statement on file for Agent #2.Plan of Correction: In the future all new board members will complete a sworn disclosure statement within the required timeframes. The board member will be contacted to complete a sworn disclosure statement as soon as possible. Administration will send evidence to the Licensing Inspector of the signed sworn disclosure statement within 10 business days.
Standard #: 63.2(17)-1721.1-B-2 Description: Based on review of board member background checks, the center failed to complete required fingerprinting within 30 days of service.
Evidence:
1. There were no fingerprints on file for Agent #1. Agent #1 began service on the board in October 2020.
2. Staff verified that there were no fingerprints on file for Agent #1.Plan of Correction: In the future all new board members will obtain all required background checks within the required timeframes. The board member will be contacted and obtain an appointment for fingerprints as soon as possible. Administration will send evidence to the Licensing Inspector of background check being completed or sent out within 10 business days.
