Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including phone contacts, documents review, and interviews.
A monitoring inspection was initiated on 11/09/21 and concluded on 11/19/2021. The director was contacted by phone and a virtual inspection was conducted. There were 89 children present, ranging in ages infant to school-age with 16 staff supervising. The director was also present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed. New requirements became effective on 10/13/21. The facility has not fully complied with the following requirement; 8VAC20-780-240: The following needs to be included in orientation prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities: Prevention of abusive head trauma. The inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Thank you for your cooperation.

Description: Based on record review and interview, the center failed to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
1. The inspector reviewed 5 staff records. Staff #5 had lived out of state and date of hire was 8/12/2021 and had lived out of state. The required out of state checks had not been completed and the director confirmed this.

Plan of Correction: The director will get the required out of state background checks completed.
