Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.

A renewal inspection was initiated on 11/30/2021. The virtual inspection of the program was conducted on 12/02/2021 and concluded with a virtual record review inspection on 12/08/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 24 children, ages 3 years-5 years, and 6 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. A total of 3 children?s records, 3 staff records and 6 board officer/agent records were reviewed.

New requirements became effective on October 13, 2021. The facility has not yet complied with the following requirement(s):
? 8VAC20-780-40-K The center shall develop written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.
? 8VAC20-780-60-A-8 Children's records shall include a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction
? 8VAC20-780-80-A For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. The center shall develop written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.

The facility is to review the new requirements and work with their assigned inspector to ensure future compliance

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 22.1-289.035-A
Description: Based on record review and discussion with staff, the center failed to ensure that all employees shall undergo a background check in accordance with the Code of Virginia prior to employment or beginning to serve as a volunteer and every five years thereafter.

Evidence: The most recent search of the central registry in the record for Staff 1 was dated 09/26/2016; there was not a 2021 repeat search available for review during the 12/08/2021 inspection.

Plan of Correction: The central registry will be mailed.
