A renewal inspection was initiated on November 12, 2021 and concluded on November 17, 2021. TA in-person inspection of the facility was conducted on November 12, 2021 with the director. There were 21 children present with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Record request was sent to the director and children's and staff records were reviewed off-site. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-3 Description: Based upon records reviewed, center did not obtain required copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him. Evidence: Staff A did not have out of state Central Registry Check completed at the time of hire (4/5/21) Plan of Correction: The director stated that she was unfamiliar with the process. She has started the process through the appropriate bureau to get the check complete.
