Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A renewal inspection was initiated on January 20, 2022 and concluded on January 24, 2022. The director was contacted by telephone and a virtual inspection was conducted. There were 33 children present, ranging in ages from 8 months to 4 years, with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records, and 3 agent records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description:	Based on a review of staff records and interview, the center did not ensure that each staff record reviewed contained a central registry finding within 30 days of employment. Evidence: 1. The record of staff #1, hired on 12/13/21, contained documentation of a central registry finding dated 1/20/22. 2. The record of staff #2, hired on 11/23/21, contained a central registry finding dated 1/8/22. 3. The record of staff #3, hired on 11/30/21, contained a central registry finding dated 1/10/22. 4. Administration acknowledged the central registries were late.
Plan of Correction:	Going forward the director/co-director will properly document email and phone calls with CPS. This documentation is found on the front of each file.

Description:	Based on a review of staff records, the center did not ensure that two or more references were checked before employment. Evidence: The record of staff #3, hired on 11/30/21, contained documentation of references that were checked and dated 12/2/21.
Plan of Correction:	Going forward, we will follow hiring steps. After interview, the assistant director/co-director should complete reference checks as well as setting up new hire fingerprints for background checks.
