Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated on January 25, 2022 and concluded on February 3, 2022. The director was contacted by telephone and a virtual inspection was conducted. There were seventeen children present, ranging in ages from infants to preschool age, with four staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #:	8VAC20-770-60-C-2
Description:	Based on review and interview, facility did not ensure 3 of 3 staff have a completed central registry search within 30 days of beginning employment. The records for staff #1 (start date November 15, 2021), staff #2 (start date November 15, 2021) and staff #3 (start date November 16, 2020) did not have a completed central registry check. Director stated the central registry searches had been sent for staff #1 and staff #2 but no results have been received and there was no documentation of mail date and any follow up on the searches. Director stated a central registry check was completed for staff #3 however they were unable to locate it.
Plan of Correction:	New central registry searches will be completed by all three staff and mailed tomorrow. Documentation will be maintained on the dates mailed and follow up will be completed and documented if results are not received within 30 days.
Standard #:	8VAC20-780-130-A
Description:	Based on review, facility did not ensure 1 of 3 children records has documentation of immunization record prior to child beginning attendance. Evidence: The record for child #3 (start date January 3, 2022) did not have an immunization record.
Plan of Correction:	Parents were told the child cannot return until they provide the immunization record.
Standard #:	8VAC20-780-60-A
Description:	Based on review, facility did not ensure 2 of 3 children records have documentation of required information. Evidence: The records for child #2 (start date October 11, 2021) and child #3 (start date January 3, 2022) did not have documentation of both parent's employment phone numbers.
Plan of Correction:	Staff will request information from parents today.
Standard #:	8VAC20-780-270-A
Description:	Based on review, facility did not ensure the areas of the center, inside and outside, are maintained in a clean, safe and operable condition. Evidence: On January 28, 2022, a 32" flat screen television was observed sitting on a bookcase in the preschool classroom, within reach of children.
Plan of Correction:	The television will be removed until it can be secured.
Standard #:	8VAC20-780-500-B
Description:	Based on observation, facility did not ensure diapers are disposed of in a disposal system that does not required the hand or diaper to touch the exterior of the surface in order to be used. Evidence: On January 28, 2022, diapers were observed in a trashcan inside the cabinet under the diaper changing table. In order to access the disposal system, staff would touch the cabinet door in order to open it and access the disposal system.
Plan of Correction:	Diaper disposal was removed from inside the cabinet during inspection and lid was ensured to be working properly with the step.
