Comments:

A renewal inspection was initiated on 01/20/2022 and concluded on 01/26/2022. The inspector conducted an unannounced inspection on-site on 01/26/2022 from approximately 2:42pm to 5:00pm. Additional documentation was submitted by the facility on 01/20/2022. On 01/26/2022, there were 161 children present with 30 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. *New requirements became effective on October 13, 2021. The facility has not yet fully complied with the following requirement: Tuberculosis screening submitted at the time of employment and prior to coming into contact with children. The facility is to review the new requirements and work with their assigned inspector to ensure future compliance.

Standard #:	22.1-289.035-B-4
Description:	Based on a review of five staff records, the center did not obtain the results of a sex offender registry check and a search of the child abuse and neglect registry or equivalent registry from any state in which one staff member had resided in the preceding five years within the required time frame. Evidence: 1) The record for Staff #3, hired on 10/13/21, indicated the staff had resided in another state outside of Virginia within the last five years. The record did not contain a sex offender registry check or a search of the child abuse and neglect registry. An out-of-state criminal background check was not required because the states participates in the National Fingerprint File (NFF) program through the FBI. 2) During interview, a member of management confirmed the required out-of-state checks were not obtained for Staff #3 within the required time frames. The sex offender registry check is required to be obtained prior to hire. The out-of-state search for founded complaints of child abuse or neglect is required to be requested within the first 30 days of being hired.
Plan of Correction:	The center will be updating their hiring paperwork to ensure deadlines are met.

Description:	Based on a review of five staff records and interviews, the center did not ensure that two staff members had a central registry finding within 30 days of employment. Evidence: 1) The record for Staff #1, hired on 10/14/21, did not contain a central registry finding. 2) The record for Staff #2, hired on 11/22/21, did not contain a central registry finding. 3) During interviews, a member of management confirmed the results of the central finding for Staff #1 and Staff #2 were not received within 30 days of employment.
