A complaint investigation was initiated on 11/04/21 and concluded on 02/08/22 in response to a complaint that was received by the department on 11/01/21 in the areas of supervision, special care provisions and emergencies. The inspector conducted an unannounced inspection on-site on 12/01/21 from approximately 3:20pm to 4:45pm. Interviews with staff were completed, documentation was reviewed, and observations were made at the center. The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint was determined to be not valid. There are no violations resulting from this inspection.
