Recommended the center to review standard 8VAC20-780-(7)-550-P-1 ( injury report documentation), 8VAC20-780-(2)-60-A ( addresses to be complete including street name , zip code, city, and state) and there needs to be a MAT trained staff when medication was provide to center and was authorized to administer at the center, the license needs to visible in the center and 8VAC20-780-70-1 Name, address and telephone number of a person to be notified in an emergency which shall be kept at the center. An to review new regulations and the updated orientation and training requirements and model forms.

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program .
A monitoring inspection was initiated on 1/10/2022 . The director was contacted by telephone to initiate the inspection. There were 26 children present and 2 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-140-A Description: Based on record review, the licensee did not ensure there was a physical exam documentation with in one month after attendance.
Evidence: The Center's record for child #1 start date 08/31/2021 did not have documentation of physical. Confirmed by director.Plan of Correction: Program Manager will ensure documents are obtained within the appropriate time frame.
