A subsidy health and safety inspection was initiated on 11/16/2022 and concluded on 11/16/22. There were 0 children present with one staff. The Inspector reviewed three child?s records and one vendor record. The physical plant was observed and the vendor was interviewed regarding policies and procedures. This announced non-mandated inspection was initiated due to an address modification. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Evidence:
