Comments:
A renewal inspection was initiated on February 7, 2022 and concluded on February 22, 2022. The site director was contacted by telephone to initiate the inspection. There were 22 children present ages four through twelve years old with three staff. The inspector emailed the site director a list of items required to complete the inspection. The Inspector reviewed three children?s records and four staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on record review the center failed to obtain a fingerprint background check prior to employment.
Evidence:
