Comments:
An unannounced monitoring inspection was initiated remotely on 2/15/22 in response to a self-report of a child being served food they are allergic to and requiring use of an epi-pen and outside medical care. Interviews were conducted and documentation was reviewed. The inspection resulted in violations and an exit meeting was conducted with the director on 3/2/22. Violations:
Standard #: 8VAC20-780-340-A
Description: Based on interview and documentation review, a staff member did not ensure the care and protection of a child they were supervising. Evidence: At approximately 3:00pm on 2/14/22, during afternoon snack/class Valentine's day party, Child A was served a "Nutri-Grain- like" bar by Staff #1. The allergy care plan on record at the center for Child A contains documentation of food allergies to wheat, eggs, peanut, dairy, and soy. Approximately 45 minutes later Child A developed a rash and threw up. Staff #1 called the director who followed Child A's allergy care plan and administered epinephrine and called 911. Child A was transported by ambulance with their parent to the hospital.

Plan of Correction: Staff # 1 was terminated immediately.

All remaining staff were retrained on Allergy Safety, Allergy Plans and State Regulations regarding allergies.

Standard #: 8VAC20-780-560-P
Description: Based on interview and review of documentation, a staff member served a child with diagnosed food allergies a food that contained ingredients identified in the written allergy care plan as allergies for the child. Evidence: At approximately 3:00pm on 2/14/22 Child A was served a "Nutri-Grain- like" bar by Staff #1 during the class party that contained wheat, soy, and milk ingredients. The allergy care plan for Child A contains documentation of diagnosed allergies to wheat, eggs, peanut, dairy, and soy.

Plan of Correction: Staff #1 was terminated immediately.

All remaining staff were retrained on Allergy Safety, Allergy Plans and State Regulations regarding allergies.
