A monitoring inspection was initiated on 3/4/22 and concluded on 3/8/22. The inspection was held from 10:10 am to 10:45 am. There were 24 children present with 3 staff supervising. The children were observed engaged in structured small group and age appropriate activities.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, medication, special care and emergencies. 3 children's records and 3 staff records were reviewed. Medications and medications authorizations were reviewed.

Standard #: 8VAC20-780-130-A Description: Based on records review, the center did not obtain documentation of immunizations for children enrolled before the first day of enrollment.
Evidence:
1. Child # 1's first day of enrollment was 10/18/21.
2. Child # 1's record did not have any documentation of immunizations on file.Plan of Correction: The immunization record for Child # 1 have been obtained.
Standard #: 8VAC20-780-140-A Description: Based on records review, the center did not obtain documentation of a physical exam for children enrolled within 30 days of enrollment.
Evidence:
1. Child # 1's first day of enrollment was 10/18/21.
2. Documentation of a physical exam was not obtained for Child # 1 to this date.Plan of Correction: The physical exam will be obtained.
Standard #: 8VAC20-780-280-B Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.
Evidence: At least three bottles of disinfectants were stored in a cabinet in the second room. The cabinet was not locked and the disinfectants were accessible to children.Plan of Correction: The cabinet will remain locked or the disinfectants will be removed to where they are not accessible to children.
Standard #: 8VAC20-780-560-G Description: Based on observation, food containers brought from home were not labeled in a way that identifies the owner.
Evidence: Three lunch bags for children brought from home did not have the children's names.Plan of Correction: Lunch bags will be checked to ensure they are labeled with the children's names.
