Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated on 12/02/2021 and concluded on 12/29/2021. The director was contacted by email to initiate the inspection. There were 3 children present and 2 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:

Standard #:	22VAC40-185-190-A
Description:	Based on review of staff records and interviews, the center failed to ensure that the program had a qualified program director. Evidence: 1. The program director did not meet the educational requirements.
Plan of Correction:	The program will ensure that a qualified person is hired as soon as possible.
Standard #:	22VAC40-185-340-D
Description:	Based on interviews with staff, the center failed to ensure that all grouping of children had a regularly present program leader. Evidence: 1. Confirmed through interviews and record review all three staff that are currently working with the children do not meet the program leader qualifications.
Plan of Correction:	All staff will obtain the 24 hours of program leader training to meet the qualifications.
Standard #:	8VAC20-770-40-D-1-a
Description:	Based on review of two staff's records, the center failed to ensure that all CPS background checks were completed within 30 days of hire. Evidence: 1. Staff # 1 was hired on 8/4/2021 and staff # 2 was hired on 6/21/21 and the CPS checks were returned on 12/6/2021. There was no documentation to indicate that the center contacted OBI to verify the status of the CPS check. The required timeframe for the CPS check is 30 days and to contact the Office Of Background Information to verify the status of the check. appropriate
Plan of Correction:	In the future the administration will follow the time frame for all background checks and will monitor and make the appropriate phone calls to verify status as needed.
