This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.
A monitoring inspection was initiated on 3/17/2022 and concluded on 3/29/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 56 children present, ranging in ages from 4 months to 5 years, with 15 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-2 Description: Based on review of four staff files, the facility failed to obtain fingerprint results prior to offering employment. Evidence: Staff A (date of hire 12/17/2019) did not have documentation of fingerprints until 10/13/2020. Plan of Correction: Fingerprint results will be obtained for all staff prior to employment.
Standard #: 8VAC20-770-60-B Description: Based on review of 4 staff files, the facility failed to have documentation of a sworn disclosure statement for each staff prior to employment. Evidence: Staff A (date of hire 12/17/2019) did not have documentation of a sworn disclosure statement on file. Plan of Correction: Each employee will complete a sworn disclosure statement upon employment.
Standard #: 8VAC20-770-60-C-2 Description: Based on review of four staff files, the facility failed to have documentation of a search of the central registry within 30 days of employment for each staff. Evidence: Staff B (date of hire 7/12/2021), Staff C (date of hire 10/19/2021), and Staff D (date of hire 1/10/2022) did not have a completed search of the central registry within 30 days of employment. There was no documentation that the requests submitted within seven calendar days of the date of employment or that the facility followed up with Office of Background Investigations. Plan of Correction: CPS checks will be sent within seven days of employment. If not returned within 30 days, staff will follow up with OBI.
