Comments:
An unannounced monitoring inspection was 03/30/2022 from approximately 9:20 am to 11:45 am. There were 24 children present, ranging in ages from 3 to 5 years, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-60-A
Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The records for Child #1 and Child #3 did not contain the address of a second designated people to call in an emergency if a parent cannot be reached.

Plan of Correction: Will inform the parent that the address is needed and monitor the parent while he/she fills in the address.

Standard #: 8VAC20-780-270-A
Description: Based on observation, equipment of the center, inside and outside, were not maintained in a safe and operable condition.
Evidence:
1) On the date of inspection, some areas of the plastic equipment on the playground was broken and had exposed pieces that could tear skin as follows: a) A broken piece near the top of the pink slide were the equipment logo had been. b) A purple "rock" on the side of the green slide.
2) Some areas near the bottom of the outside wall of the center (on the playground) were broken and showed the stucco starting to come off.
3) The bottom area of some walls of the stalls in the bathrooms showed rust.

Plan of Correction: #1-The broken areas were covered with tape as instructed by the inspector.
#2-The owner will inform John Nichols, property manager, of needed repairs.
#3- "U" shaped repair structures will be added to the walls to cover rusted areas.

Standard #: 8VAC20-780-330-B
Description: Based on observation, resilient surfacing in the playground did not comply with with minimum safety standards.
Evidence:
1) On the date of inspection, there was not enough resilient surfacing remaining on the playground. Dirt and ground was able to been seen from where the resilient surfacing has been kicked out.

Plan of Correction: Resilient surfacing will be purchased and applied to the playground.
